We will aim to collaborate and work in a streamlined way to strengthen relationships within our PCN and build effective community links whilst responding to local, regional and national initiatives.  The projects in the PCN involve working with outreach teams in the local community.  This work involves working with the local food banks, offering health checks, lifestyle advice, social prescribing support and assistance and working to tackle health inequalities in the area.

This online service does require you to bring in ID into the surgery to register for the service. But first you need to register. Please enquire about registering at the surgery reception.
